The Transport Accident Investigation Commission (TAIC) has launched an investigation into an explosion onboard the Malta-registered bulk carrier M.V. Olivia which occurred on 24 May 2025 in South Port, Bluff, New Zealand.
The incident occurred in the vessel’s engine room while three crew members were working on a boiler. A reported explosion or flashback during the maintenance operation resulted in injuries to all three individuals. Details on the severity of the injuries have not been publicly released.
TAIC has classified the event as a “serious marine casualty,” in line with the requirements set out by the International Maritime Organisation’s Casualty Investigation Code. The commission is currently gathering evidence and interviewing witnesses as part of its formal inquiry.
Currently, the vessel is berthed at South Port in Bluff, New Zealand. Further updates will be provided as the investigation progresses.
